Symptom diary
The diary that fills in its own context.
Log a symptom, a sick day or a flare in a few taps, on your iPhone or straight from your Apple Watch. Sam pulls in what Apple Health already recorded around that day - sleep, HRV, resting heart rate and activity, next to the drinking days you logged yourself - so the entry arrives with context instead of just a word. Everything rolls into a monthly report you can bring to your appointment.

What you can log
One place for symptoms, sick days and the diary your condition actually needs. Sam keeps the record; your doctor reads it.
Symptoms, in a few taps
Note what you felt, how strong it was and how long it lasted. Add a line of detail when it matters, or keep it to one tap on the Watch when it doesn't.
Sick days and days you drank
Mark a day as a sick day and it stays on the timeline with the rest. Drinking days log the same way, so the pattern is visible over months rather than remembered in fragments.
A diary shaped by your condition
An attack diary for migraine, a seizure diary for epilepsy, a flare log for gout, and so on across Sam's 8 Medical Programs. Acute medication is logged with the entry, so the dose and the episode stay together.
Context you never have to write down
This is what a printed template cannot do. When you log a day, Sam attaches what Apple Health already recorded around it - sleep, HRV, resting heart rate, steps, workouts - so the entry carries the week it came from.
How the symptom diary works
- 1
Log the day
iPhone or Apple Watch, a few seconds either way. Symptom, sick day, flare, attack - whichever your program tracks.
- 2
Sam fills in the rest
The six Apple Health signals your program follows are attached to the entry automatically. No typing, no remembering last Tuesday's sleep.
- 3
Bring the report
Entries roll up into a monthly PDF, doctor-friendly and printable, with the year-on-year view underneath.
Sam records what you log - it doesn't judge it.
Sam is a wellness companion, not a medical device. It does not diagnose conditions, interpret your readings, or tell you whether a treatment is working. Consult a qualified healthcare professional for medical decisions.
Questions about the symptom diary
I was looking for a printable symptom diary template. Is this that?+
It replaces one. A paper template needs you to fill in every column, including the numbers you don't have. Sam logs the day in seconds and attaches the Apple Health context itself, then gives you a printable monthly PDF at the end - the output a template is trying to get you to.
Does it cover my condition specifically?+
Sam has 8 Medical Programs spanning 15 condition families, and each one brings its own diary: attack entries for migraine, seizure entries for epilepsy, flare entries for gout, symptom scores for reflux, hay fever and asthma. Six Apple Health signals are tracked alongside, chosen per program.
Can I log from the Apple Watch?+
Yes. A symptom, a sick day or an attack can be logged from the wrist, which is usually where you are when it happens. It syncs to the iPhone and lands on the same timeline.
What do I actually take to the appointment?+
A monthly PDF report: your logged days, your validated monthly instrument score where your program has one, and the Apple Health context around it. It is built to be printed or read on screen in a short consultation.
Does Sam tell me what my entries mean?+
No, and that line is deliberate. Sam organizes and presents what you logged. It does not interpret symptoms, suggest a diagnosis, or advise on medication - that is your doctor's work, and the report exists to make it easier.
